The EHA Population Health program gives you access to tools and resources that will simplify health care and offer guidance, so you get the support you need. Our goal is to help you live a healthy and happier life and lower your health care costs.
This program connects you to a nurse care manager who works with you to create a personalized care plan that can help you:
- Lower health risks related to diabetes, high blood pressure and other conditions
- Navigate the health care system, so you get the most from your health insurance plan
- Maintain a healthy weight
- Meet your health and wellness goals to live a healthier and happier life
- Have a healthy pregnancy
- Manage stress and mental health
How does the program work?
The EHA Population Health program is provided through an easy-to-use mobile app, where you can track your progress, access educational information and get answers and guidance from your nurse health coach. Together, you and your nurse health coach will create a personalized care plan based on your health and wellness goals — taking your specific plan benefits and costs into account.
