Note: Vitality principal ID number is a combination of the following information:
- Last five digits of your Social Security Number (ex. 01234)
- 10-digit birthday – YYYYMMDD (19850102)

After you have created your account, complete the Vitality Health Review. It takes approximately 10 minutes to answer questions about your overall health, physical activity, eating habits and more. When it’s complete, you’ll receive your scientifically calculated Vitality Age®: a unique measure of your “true” age based on your lifestyle behaviors.
Download the Vitality app
You can also use your mobile device to interact with Vitality. Download the Vitality TodayTM app and you can capture and submit athletic events, sports leagues, gym workouts and prevention activities to earn points.

Visit myNebraskaBlue.com and log in or create your myNebraskaBlue account. Select Vitality Wellness in the Find Care and Estimates section at the bottom of your myNebraskaBlue homepage. This link will take you directly to your Vitality account.
