Care Management

Whether you were recently hospitalized, getting treatment for cancer, or just need a little extra help with your health, our nurse care managers work one-on-one with you to get you the care you need.

Our nurse care managers can help:

• Get you on the road to recovery – helping you manage pain or wound care to get you moving again.
• Navigate the complexities of the health care system – help you understand your insurance, guide you to resources and help you make informed decisions.
• Guide you as you face major life events, such as cancer treatments, traumatic injuries or even finding the right support for children with special needs.

Our nurse care managers will advocate and coordinate your care to make sure you get the support you need.

Care managers will:

• Help you understand your medical condition
• Assist with medication management
• Schedule follow-up appointments
• Teach on warning signs and worsening conditions
• Help you maintain personal health records
• Provide information to help you understand their health care benefits
• Identify and refer services and resources to meet your needs
• Coordinate care plans with you, health care providers and any other relevant caregivers (including family members)
• Encourage involvement in your treatment program and empower you to manage your condition
• Consistently communicate with you to assure your needs are being met
