Blue Cross and Blue Shield of Nebraska (BCBSNE) is partnering with NxtCare to advance a shared post-acute strategy focused on improving patient outcomes and optimizing care quality for members across Nebraska. This collaboration brings together BCBSNE's extensive network and NxtCare's innovative care-coordination technology.
BCBSNE and NxtCare will work with post-acute providers to share meaningful performance insights that reflect clinical quality, patient satisfaction, and care coordination. The goal is to highlight strengths and opportunities that support continuous improvement across the post-acute care continuum.
Providers who demonstrate strong performance and deliver positive member experience will be recognized through our quality-focused selection process. KPIs set by NxtCare and BCBSNE determine whether post-acute and home health providers meet performance goals and qualify for the high-performing tier. NxtCare will also integrate a tool into the discharge process to help identify home health agencies that offer the appropriate level of care for each member.
NxtCare will work with the discharge teams to develop post-acute plan of care for each BCBSNE patient.
Members will be connected with providers who excel in care coordination throughout recovery, ensuring an enhanced patient experience. Our shared focus and strategy surrounds connecting members to providers who excel in clinical quality and care coordination, creating seamless care transitions.
The partnership utilizes continuous monitoring and performance insights to maintain the highest standards of care delivery. NxtCare's technology platform enables real-time tracking of quality metrics, patient satisfaction scores, and coordination effectiveness.
In the coming weeks, teams from BCBSNE and NxtCare will be reaching out to collaborate with hospital case management teams, post-acute provider teams, and other key stakeholders as we advance this post-acute strategy together.
