Preauthorization Appeal Process
To appeal a denied preauthorization, providers must complete the Appeal Request Form and submit it manually to Blue Cross and Blue Shield of Nebraska. The form is available on our Provider Forms page.
These appeals cannot be submitted through NaviNet®.
