Agent of Record Changes and Appointment Guidelines

Follow the instructions below for agent/agency appointments or to change the Agent of Record (AOR) on a policy or book of business.

Guidelines

New Sales
A new Armor policy will be loaded under the agent writing number that accepted the offer for the members. AOR changes will not be processed for new Armor sales after the offer has been accepted on the Broker Business Portal. 

These requests will be processed during the initial enrollment of the applications that are processed within 5-10 business days of receipt.

Re-enrollment
An Agent of Record change can be requested for any of the following circumstances for an Armor Health re-enrollment offer:

  • A member would like to change the agent they are working with to a different agent
  • An agent has changed agencies and they would like to move the policy to their new agency
  • An agency would like to change the agent that is working on the policy within their agency
  • A re-enrollment offer is not showing on the Broker Business Portal under an agents’ writing number
Instructions

Download the AOR form and fill out completely including:

  • All applicant information
  • All broker information
  • Member signature
  • Agent signature
  • Current date

The AOR form must be signed and emailed to Broker_Appointment@NebraskaBlue.com within 90 days and at least five days prior to the re-enrollment effective date. If the AOR request is received outside of this timeframe, it will not be processed. 

The commissions effective date will be the same effective date of the re-enrollment offer.

The AOR change will be processed within 2-3 business days of receipt. Once processed, a confirmation email will be sent to the agent/person who submitted the request.

**Please note that if a re-enrollment offer has already been accepted on the Broker Business Portal by another agent and agency, the AOR change will not be processed. **

 
 

 

Guidelines

AOR changes will only be accepted with a dental essentials plan change. For example, if a member moves from the Preventive Plus plan to the Premier plan.

Download and complete the DentalEssentials application. Return the completed application via mail, email or fax.

Guidelines

An agent of record change will occur automatically when a member re-enrolls into another plan. Resubmitting an application for a member into the same plan they are already enrolled in will not change the AOR.

 

Guidelines

AOR changes will only be accepted with a plan change requiring medical underwriting. (i.e., applying for a discount or changing from one plan to another that would require underwriting). Note: The AOR Change Request must coincide with the effective date of the plan change and be submitted with the plan change for the AOR to be updated on the account. 

Instructions
  1. Submit an application for the member's plan change through DRX – the online enrollment portal
  2.  Download the Medicare Supplement AOR form and fill out completely including:
    • All applicant information
    • All broker information
    • Member signature
    • Agent signature
  3. Prior to the plan change effective date, the AOR form must be submitted by uploading with the DRX application or via fax/email to

Requests will be processed with the change application within 2-3 business days of receipt. 

**Please note that if the application is denied by underwriting, the AOR change will not be processed. **

Instructions
  1. Download the AOR form and fill out completely including:
    • All Group information
    • All broker information
    • Effective date
    • Group decision maker signature
    • Current date
  2. Email the completed form to Broker_Appointment@NebraskaBlue.com. A signed letter on letterhead from the group will be accepted in place of the signed AOR form if it includes all information required on the AOR form. 
  3. The AOR change will be the first of the month following receipt of the completed form or letter.  

AOR changes will be processed within 1-2 business days of receipt. A rescind letter will be sent to the prior AOR and a 10-business day rescind period will begin. After the rescind period is complete, the AOR will be updated in our systems and on the Broker Business Portal.

 
 

 

New Agency Appointments
  1. Download the required appointment documents below, fill out the agency information completely and sign. This includes:
  2. Send the completed, signed documents to Broker_Appointment@NebraskaBlue.com

The appointment will be processed within 2-3 business days of receipt of the required documentation. Once approved, the writing number and sales contact information will be sent to the agency. 

**Please note that if the new agent has a prior appointment on file, we will have to contact the agent to verify which agency they want to be under. This will add to the time the appointment will take to be processed. **

New Agent Appointments
  1. Download the Appointed Agent Form, fill out completely and sign.
    • Section I – Agency Information
    • Section II – Agent(s) Information
    • Section IV – Title Holder SignatureErrors and Omissions Insurance Certificate (if Applicable)
       
  2. Send the completed, signed documents to Broker_Appointment@NebraskaBlue.com

The appointment will be processed within 1-2 business days of receipt of the required documentation. Once approved, the writing number and sales contact information will be sent to the agency. If the new agent has a prior appointment on file, we will contact the agent to verify which agency they want to be under. This will add to the time the appointment will take to be processed. 

**Certification training will may not be completed until the agent is appointed on the DOI. BCBSNE will appoint agents within 15-30 days of set; during AEP, we will appoint agents weekly on the DOI.**

Follow the instructions below to request a book of business (BOB) transfer:

  1. Submit a letter on current agency letterhead requesting the BOB transfer including
    • Current agent/agency information
    • Agent/agency the business is being moved to 
      An agency can request a BOB transfer within their own agency by submitting all information above in an email format from a stakeholder in the agency
  2. Submit the request to Broker_Appointment@NebraskaBlue.com 30 days in advance of the requested effective date
  3. The transfer will be effective the first of the month following the 30-day notice

**Please note that all business is owned by the current agency where the business resides, so all BOB transfers will need to be released from the current agency. This could add time to the process if the prior agency does not release the business right away.**
